Terms and conditions for the program "Meals of Life"

1. Consent to the following conditions and their acceptance is necessary in order to submit the application and participate in the "Meals of Life" program carried out by the Hellenic Association of Women with Breast Cancer "Alma Zois".
2. I voluntarily participate in the "Meals of Life" program organized by the Hellenic Association of Women with Breast Cancer "Alma Zois".
3. I understand that the program aims to support female breast cancer patients who live in Athens alone or with their minor children and are or are about to start chemotherapy or undergo surgery for this disease.
4. The information I submit in the application form for participating in the program is true
5. I consent to the storage / processing of my personal data (name, address, hospital/doctor's opinion, E1 income tax document, family status certificate, identity card) by the Hellenic Association of Women with Breast Cancer "Alma Zois" and to their sharing with the Meals providing Company for the preparation and distribution of meals for as long as I participate in the Program.
6. Submitting the application form for participation in the program does not automatically imply the acceptance of my participation in it by the Association, but it is a necessary condition
7. I understand that my application will be evaluated by the Hellenic Association of Women with Breast Cancer "Alma Zois" based on specific criteria (place of residence, family status, certificate of treatments).
8. The Hellenic Association of Women with Breast Cancer "Alma Zois" reserves the right to change the criteria for the inclusion of new participants in the program, without prior notice
9. The "Meals for Life" program includes two main meals for a period of 5 calendar days from the day of treatment or surgery.
10. The program covers deliveries to all Municipalities of Attica, except for the following:
    Agioi Anargyroi-Kamatero, Aegina, Aspropyrgos, Vari-Voula-Vouliagmeni, Elefsina, Kropias, Lavreotiki, Mandra-Idyllia, Marathon, Markopoulou Mesogaias, Megareon,
    Islands of the Prefecture of Attica, Paenia, Rafina-Pikermio, Salamina, Saronic, Spaton-Artemidos, Fili, Oropos
11. I will be informed about the outcome of my application within 7 working days from the moment of submission
12. After joining the program, I understand that the Meals preparation and distribution company will contact me with the aim of listing my dietary preferences and restrictions, defining the distribution schedule and other details about my participation in the program.
13. I am solely responsible for informing the nutritionist about any allergies, intolerances and other food restrictions.
14. Meals are delivered from Monday to Friday between 09:00 and 13:00 upon consultation with the Meals preparation and distribution company, to the address stated in my application
15. I reserve the right to terminate my participation in the program upon written notification of the Association
16. I agree that after the completion of my participation in the Program I have no other claim from the Panhellenic Association of Women with Breast Cancer "Alma Zois"
17. I consent to the retention of my personal information for 15 days after the completion of my participation in the Program, provided that it is then destroyed.
18. I understand that any communication, suggestion, complaint or evaluation regarding the program needs to be made by me within 15 days after the completion of my participation in the program
19. After completing my participation in the program, I consent to the completion of an anonymous evaluation form for it and to the provision of a short phrase (quote) aimed at promoting the program, without mentioning personal information
20. The Association reserves the right to postpone / readjust the program in case of extreme weather conditions or force majeure events, always after informing the participant
21. The Association reserves the right to stop the program for a serious reason at any time, declaring its intention in writing to the beneficiary
