4. Health data: medical history of disease, type of treatment, type and date of surgery.

The above information is included in the physical and electronic archive of the "Alma Zois" Association, which has unique purposes: a) the processing and identification of those requesting services from "Alma Zois" and b) the best service to those served and is used only for convenience during their communication with the "Alma Zois" Association. Only authorized persons who work or collaborate with the "Alma Zois" Association have access to this archive. The "Alma Zois" Association guarantees that your personal data will not be used for purposes other than those mentioned in this form, without having informed you in advance and without your relevant approval where required. Your data is kept for as long as you are a member, after which it is archived anonymously and stored in a secure location with controlled access. "Alma Zois" Association implements appropriate technical and organizational measures to ensure the legal and appropriate use of the Personal Data it receives, its integrity, confidentiality, availability, security and protection and ensures the prevention of any unauthorized access to such data.

You have the right to withdraw your consent to the use of your personal data whenever you wish. In this case, the legality of the processing carried out while your consent was in force is not affected. You can request a copy of your data, its correction, its deletion, restriction of its processing or object to such processing, as well as exercise your right to portability by submitting a relevant request, in accordance with the corresponding articles 13-21 of the European of the General Data Protection Regulation (679/2016).
